ETHOS

I've been straining heavily and have minor anal fissures.

Patient

Hi, I'm experiencing some discomfort and I'm not quite sure what to do about it. I was hoping you could give me some advice.

Triage Assistant

Sure, I can help with that. Can you please tell me where it hurts?

Patient

The discomfort is primarily in my lower abdomen and it's quite uncomfortable.

Triage Assistant

Let's get started with a few questions to assess your situation better. Where exactly does it hurt?

Patient

Most of the discomfort is towards my lower abdomen, but it's also affecting another area a bit further back.

Triage Assistant

Does the pain shoot anywhere else, such as your back or chest?

Patient

No, it doesn't really shoot anywhere. It mostly stays in those areas.

Triage Assistant

When did the pain begin? Was it minutes, hours, or days ago?

Patient

It's been going on for a few days now. It seems to get a bit worse every day.

Triage Assistant

Has the onset of pain been gradual or sudden?

Patient

It's been more of a gradual onset rather than a sudden one.

Triage Assistant

Is the pain constant, or does it come and go? If it comes and goes, how long does it last each time?

Patient

It's not constant. The pain comes and goes. When it comes, it can last for quite a while, sometimes hours.

Triage Assistant

Do you have any pain right now?

Patient

Yes, I'm feeling some discomfort right now. It's not as bad as it gets sometimes, but it's definitely there.

Triage Assistant

How would you rate your pain on a scale from 1 to 10, where 1-3 is mild (doesn't interfere with normal activities, abdomen soft and not tender to touch), 4-7 is moderate (interferes with normal activities or awakens you from sleep, abdomen tender to touch), and 8-10 is severe (excruciating pain, doubled over, unable to do any normal activities)?

Patient

I'd rate it around a 5 or 6 right now. It's enough to interfere with my activities, but I can still manage to get things done, albeit with some difficulty.

Triage Assistant

Have you ever had this type of stomach pain before? If yes, when was the last time and what happened that time?

Patient

No, I haven't experienced this type of pain before. This is the first time I'm dealing with something like this.

Triage Assistant

What do you think might be causing the stomach pain?
